Diabetes testing in Lucknow: which test, how often, and what the numbers mean

Diabetes is common enough in north India that most families have someone managing it, and common enough that most people over forty should know their numbers. The confusion is rarely about whether to test — it is about which of the four sugar tests is the right one.
They are not interchangeable. One diagnoses, one monitors, one catches what the others miss, and one is mostly used in pregnancy.
The four tests, and what each is for
Fasting plasma glucose is a single snapshot after eight to ten hours without food. Post-prandial glucose is taken two hours after a meal and shows how well your body handles a sugar load. HbA1c reflects average blood sugar over roughly the previous three months and needs no fasting at all. An OGTT is a formal challenge test, used where the picture is unclear and routinely in pregnancy.
For diagnosis, fasting glucose and HbA1c are the usual starting pair. For monitoring someone already diagnosed, HbA1c is the workhorse, typically every three to six months as the doctor advises.
- Fasting glucose — normal below 100 mg/dL; 100–125 is pre-diabetes; 126 and above on two occasions suggests diabetes
- HbA1c — below 5.7% normal; 5.7–6.4% pre-diabetes; 6.5% and above suggests diabetes
- Post-prandial (2 hour) — below 140 mg/dL normal; 140–199 impaired; 200 and above suggests diabetes
- These are the widely used thresholds; diagnosis is your doctor's, made on repeat testing and your clinical picture, not on one reading
Where HbA1c can mislead
HbA1c measures sugar attached to haemoglobin, so anything that changes red cell lifespan changes the result. In anaemia — which is common here — in recent blood loss or transfusion, in some haemoglobin variants, and in advanced kidney or liver disease, HbA1c can read falsely high or falsely low.
This is a practical point, not a technicality: if your HbA1c and your glucose readings disagree, that discrepancy is information, and a CBC alongside is often what explains it.
What else matters once you have diabetes
Sugar control is one part of it. The annual checks that prevent the complications people actually fear are kidney function with urine albumin-creatinine ratio, a lipid profile, blood pressure, a dilated eye examination and a foot check.
Those are the tests that catch damage early enough to do something about. An HbA1c every three months with none of these is monitoring half the disease.
